The best diagnostic method (even it is an expensive and hardly accessible technique). Uterine fibroid is isointense on T1-weighted, iso-hypointense on T2-weighted images, it is sharply contoured, its contrast enhancement is usually weak.Signal intensity might be higher on T2 due to venous dilation, lymphatic vessel dilation, edema, increased cellularity, internal bleeding (mostly in gravidity or if contraceptive pills are taken), hyalinal, myxoid degeneration. Its contrast enhancement failes both in time and in intensity from the muscularity of the uterus. We need to think of leiomyosarcoma in case of fast increase after menopausa – even if the depravity is rare –. Infiltration of the sorrounding areas is always informative (but MRI does not tilt, only the histology!).
