III./4.4.: Clinical course
The natural course of the disease is dependent on the VSD and on the absence or presence of pulmonary stenosis or atresia. Long terms survival is rare without intervention, majority of the children become symptomatic at early on, and their life expectancy is limited without surgical intervention.
III./4.5.: Tests
III./4.5.1.: Physical exam
|
|
-
- general condition of the newborn, skin color, respiration;
-
- cardiac auscultation of rhythm and murmurs, recognition of abnormal heart pounding;
-
- comparative exam of peripheral vessels;
-
- blood pressure monitoring on all extremities;
-
- checking signs of circulatory failure (tachypnea, dyspnea, edema, cardiomegaly, arrhythmia).
III./4.5.2.: EKG
1. DORV with pulmonary stenosis
-
- right deviation;
-
- right atrial load;
-
- right ventricular hypertrophy or right bundle branch block.
2. DORV without pulmonary stenosis
-
- „Superior” QRS axis;
-
- Left atrial load;
-
- biventricular hypertrophy;
-
- grade AV-block.
3. Taussig-Bing-anomaly
-
- right deviation;
-
- right atrial load;
-
- right and/or left ventricular hypertrophy;
-
- grade AV- block.
